The ASHP Pharmacy Practice Sections and New Practitioners Forum (NPF) Distinguished Service Award recognizes a member from each Section and NPF whose volunteer activities have supported the mission of their Section or NPF and helped advance the profession.
Submissions of nominations must be received no later than April 26, 2010.
Nominations should be accompanied by:
- a complete Nomination Form
- a one to two page statement about why the individual is qualified for the award [submitted separately via e-mail to sections@ashp.org]
- a copy of the nominee’s curriculum vitae [submitted separately via e-mail to sections@ashp.org]
